When to Consider Sports or School Physical

Many schools, athletic programs, and youth organizations require a current physical before students can participate in classes, athletics, or other organized activities. These examinations help confirm that students are healthy enough to safely participate while identifying any concerns that may require additional evaluation.

A sports or school physical may be required for:

School enrollment

Youth sports leagues

Recreational sports and activities

School athletic programs

Summer camps

Other school-sponsored programs

Why It's Best to Schedule Your Physical Early

Scheduling your child's physical before the school year or sports season begins can help avoid last-minute stress and ensure there is enough time to address any concerns before participation deadlines.

Scheduling early also allows you to:

Complete required paperwork before school or league deadlines

Allow time for additional evaluation if recommended

Avoid the seasonal rush before sports tryouts and school registration

Help your child start the season prepared and ready to participate

Planning ahead gives families greater flexibility and helps ensure students are ready when practices, games, or classes begin.

What to Bring for Sports or School Physical

Bringing the appropriate paperwork and information helps your visit go more smoothly and allows our providers to complete your child's physical as efficiently as possible.

Please bring:

Required school or sports physical forms

A photo ID when applicable

Insurance card, if using insurance

A list of current medications

Eyeglasses or contact lenses, if worn

Information about medical conditions, allergies, or previous surgeries

Immunization records, if required by your school or organization

Whenever possible, a parent or legal guardian should accompany minors to the appointment.

What the Sports or School Physical May Include

Every student's health needs are different, so the examination is tailored to your child’s age, medical history, and the requirements of their school or athletic organization.

Your child's physical may include:

Review of medical history

Blood pressure and other vital signs

Heart and lung examination

Discussion of previous injuries, medical conditions, or medications when appropriate

Height and weight measurements

Vision screening

Ear, nose, and throat evaluation

Assessment of posture, joints, flexibility, and strength

The purpose of the examination is to help determine whether your child can safely participate in school or athletic activities and to identify any concerns that may require additional evaluation.

School Forms & Medical Clearance

As part of your visit, our providers complete the required documentation based on the findings of your child's examination and the specific requirements of the school, sports league, or organization.

If your child is medically cleared, the necessary forms can typically be completed during the visit. If the examination identifies a condition that requires additional evaluation or specialist clearance before participation, we explain our findings, discuss the next steps, and provide guidance on obtaining any additional documentation that may be needed.

When We May Recommend an EKG

Most students do not need an electrocardiogram (EKG) as part of a routine sports or school physical. However, in certain situations, additional cardiac evaluation may be appropriate.

An on-site EKG may be recommended if your child's:

Medical history raises concerns about heart health

Symptoms warrant additional cardiac evaluation

Physical examination suggests further assessment is appropriate

The decision to perform an EKG is based on your child's medical history, symptoms, and clinical evaluation. It is not a routine part of every sport or school physical.

What to Expect During Your Visit

Our goal is to make your child's visit convenient, efficient, and informative while ensuring all required forms are completed whenever appropriate.

Step 1: Medical History Review

Your provider will discuss your child's medical history, current medications, allergies, previous injuries or surgeries, and any concerns related to participating in school or sports activities.

Step 2: Physical Examination

A comprehensive physical examination will be performed based on your child's age, health history, and the requirements of the school or athletic program.

Step 3: Forms & Next Steps

If your child is medically cleared, we complete the required forms during your visit. If additional evaluation is recommended, we explain the next steps before participation.
